If lying flat makes the throat burn, if you wake in the small hours already coughing, or if a second pillow quietly turned into a third — this is what happens while you sleep.

Read This If You Have Started Sleeping Sitting Up

Three pillows. A cough at two in the morning that will not stop. A husband who moved to the spare room. I want to tell you what was happening, because for four years nobody told me.

Nobody buys a third pillow on purpose. It happens slowly.

“The culprit is not the acid. It is the pepsin.”

Dr. Jamie Koufman, ENT surgeon, who studied 899 reflux patients

Pepsin is a little tool your stomach makes to chop food up. It belongs in your stomach. Lying flat is how it gets out.

The account below is written from the experiences of people who live with reflux, including verified customer reviews. Names and identifying details have been changed. Every study named in it is listed in full at the end of this page, with its authors and its journal.

Start with the pillows. I want to start with the pillows, because that is how you know.

The throat that starts burning the moment the head goes flat…

The cough that arrives around two in the morning, before you are properly awake…

The sour taste that water does not shift…

The second pillow that quietly became a third…

Four things crossed out - acid pills, cutting foods out, more pillows, throat sprays
Every one of these is aimed at the chest. The problem is higher up.

Not one of those is a sleep problem. That is why it goes unnamed for years.

A bedside lamp switched on in a dark bedroom beside an unmade bed in the middle of the night
The stretch of the night this happens in.

None of it needs panicking about. But there is a name for the stage in between, and almost nobody gets told it.

A few things are worth a doctor rather than an article. If food sticks going down, if swallowing hurts, if you are losing weight without trying, if you ever bring up blood, or if you wake unable to pull air in, get that looked at. Those are not the stage this is about.

It is the throat stage. Lying flat takes gravity out of the equation, so the stomach sends something up, it goes past the chest, and it lands in the throat. There is no burning in the chest because the chest is not where it is stopping any more.

That is also why what finally worked for me was a liquid taken at night, not a tablet.

I know what it is like to work out the exact angle of pillows that lets you sleep at all.

“I know waking at two in the morning already coughing, before being properly awake”

I know being told to stop eating late, and doing it, and nothing changing.

You will notice I keep saying I know. That is not from reading about it. It is from four years of it.

A part drunk glass of water on a bedside table with wet rings left on the wood beside it
Picked up and put down again, all night.

Four Years, In Four Nights

Year one. I picked up a second pillow at the store and told myself it was for my neck.

Four pillows stacked upright against a headboard forming a steep backrest, the bed empty
Nobody buys the second one on purpose either.

Year two, March. My husband carried his pillow into the spare room and said it was his back. It was not his back.

A duvet and one pillow left on a living room armchair with a blanket trailing onto the carpet
The spare room was not about his back.

Year two, autumn. I gave up the early shift at work, because I knew what I would feel like at seven. It felt sensible at the time. They all did.

Year three, the night I still think about. I woke up choking. I could not pull air in for two or three seconds. I sat on the edge of the bed until it got light.

Year four, my sister’s birthday lunch. She booked a week away for the two of us and I said no. I could not work out how to explain the pillows.

That was the week I stopped calling myself a bad sleeper.

“I had stopped thinking of it as a problem. I thought of it as my life”

Then a throat surgeon told me one thing in about two minutes, and it was not what I had been told for four years.

It was fixable. It had a cause, the cause had a name, and the name was not on a single box in my cupboard.

  1. The same woman propped upright on a stack of three pillows, awake and tense in the middle of the nightThree Pillows
  2. The same woman in the same bed with two pillows behind her, settled and starting to sleepTwo Pillows
  3. The same woman lying lower in the same bed, still two pillows, sleeping through the nightStill Two Pillows
  4. The same woman lying flat on a single pillow, arms loose at her sides, properly asleepOne Pillow
The stack comes down one pillow at a time, and the spare ones end up on the chair. That is the shape of it.

* One person’s experience, drawn as a picture. It is not a promise of what anyone else will see.

That liquid is swallowed slowly, once, after the evening meal, and it goes to work while you are lying down. I will come to what is in it. First, why lying down is the whole problem.

Why It Happens At Night And Not During The Day

Here is the thing I did not understand for years, and it is almost stupidly simple.

Gravity is the only thing keeping your stomach contents down.

All day you are upright.

Whatever wants to travel up has to fight against gravity to do it, and mostly it loses.

Then you lie down flat, and the one thing that was holding the line is gone.

“That is why it is a night problem”

It is not that your body is worse at night.

It is that lying flat removes your protection, and the pillows are you working that out on your own without anyone explaining it.

You were doing the right thing for the right reason.

You did not know why, and it was never going to be enough.

The Part About What Is Actually Burning

I always assumed it was acid.

Everyone does.

The name says acid.

Your stomach makes acid, yes.

It also makes an enzyme called pepsin.

An enzyme is a protein that takes other proteins apart, and pepsin is the one that breaks down meat and eggs and fish.

It cannot tell the difference between your dinner and you.

It is built for that one job.

Your stomach is lined to cope with it.

Your throat is not, so when you lie flat and things travel up, pepsin comes too, and it settles on tissue that has no protection at all.

Your stomach rebuilds that green layer every day. Your throat has no way to make one.

Here is the way the surgeon explained it to me, and it is the only way it ever made sense.

Acid is the match. Pepsin is the gasoline, and it is already soaked into your throat.

A match burning out on its own beside a square of cloth soaked through and still alight
One goes out on its own. One has soaked in.

A match on its own burns out. Gasoline that is already soaked in does not.

That is what pepsin does.

It is also why the pills helped a little and never finished the job. They took away some of the matches. The gasoline was still sitting there.

It stays there.

It does not simply wash away when you sit up in the morning.

Dr. Jamie Koufman, the ear nose and throat surgeon who has spent her career on this condition, says it plainly.

The culprit is not the acid.

It is the pepsin.

“The culprit is not the acid. It is the pepsin.”
Dr. Jamie Koufman, ear nose and throat surgeon

And this is why the pills so many of us are given never fix the nights. Acid blockers lower acid. They do not switch off pepsin. Three researchers in England went back over the evidence on this in 2012, and they put the answer in the title of their own paper. The thing doing the damage is the pepsin.

The pill does its job and the enzyme carries on.

An open kitchen drawer crowded with the real medicine packs most people already keep at home - Prilosec OTC, Nexium 24HR, Pepcid AC, Gaviscon Extra Strength, Flonase Allergy Relief, Mucinex DM, a Tums bottle, Halls cough drops and Ricola throat drops - with loose tablets and crumbs
Not one of these is a bad product. Every one of them is built for a chest or a nose.
A crowded bathroom shelf lined with half used unlabelled throat and nasal spray bottles
A shelf of things aimed at a nose, for a problem that is not in the nose.
Everything That Was Already In The Cupboard
  • Tums and Rolaids — chewed at midnight, good for about 40 minutes
  • Omeprazole — taken every morning for eight months
  • Famotidine — the one taken at night instead
  • Apple cider vinegar — a spoonful before dinner, because the internet said so
  • Flonase and allergy tablets — two years of them, for a nose problem that was not there
  • Cutting out coffee, then tomatoes, then onions, then wine — one at a time, over two years
  • The wedge pillow — used for a month, still in the loft

Some of those genuinely helped, and that is the part worth being honest about. They helped for an hour. An hour is not a night, and a night is what the throat needed.

A triangular foam bed wedge still sealed in dusty plastic, propped against boxes in a loft
Bought, tried, and put away still in its wrapper.
Past the chest, where the burning would have warned you, and onto the one piece of tissue with no nerve endings for it.

Here is the part that took four years to arrive, and it is the only part that ever mattered.

The lining of the throat was never failing to heal. It was healing, a little, every single day. And then it was being hit again the same night.

That is why everything helped for an hour and nothing held for a month. Not one of those products was wrong. Every one of them was aimed at the burning, and the burning was never what was causing the harm.

Being told that made me angry first and hopeful second. Angry because it is not complicated and nobody said it. Hopeful because it means nothing that failed, failed because of the person taking it.

Two slices of tissue side by side, one puffed up and angry red, the other settled flat and calm pink
Tissue that is still angry cannot get on with rebuilding.
Where This Usually Starts, And Where It Usually Goes

A general picture of how throat reflux tends to move when nothing about the nights changes. Individual experience varies.

Year one
Clearing the throat. Put down to a cold that hung around.
Years two to three
Mucus every morning. A voice that goes by the evening.
Years four to five
A lump that will not swallow away. A second pillow.
Year six onward
Sleeping propped upright. The food list down to a handful.
Every night it gets hit, so it never finishes healing. The layer is the night off.
What Keeps Failing
  • A second pillow, then a third
  • Sleeping half sitting up in a chair
  • Antacids at bedtime that wear off by two in the morning
What the Throat Actually Needs
  • Lays a coat over the raw lining
  • Calms the swelling underneath it
  • Feeds the lining while it rebuilds
  • One 2 ml serving after the evening meal

If You Are Reading This About Someone Else

I want to say something to the person who is not the one coughing.

If you are lying awake listening to someone you love wake up choking, and you have started sleeping somewhere else so that at least one of you gets a night, you are not being unkind.

You are exhausted.

Two people lose these nights, not one.

You are also usually the one who ends up looking for an answer, because the person in it has been told it is nothing so many times that they have stopped asking.

What Finally Changed The Nights

Coat it. Calm it. Heal it. Seven ingredients doing three jobs.

You cannot scrub gasoline out of cloth it has soaked into.

What you can do is lay something over it so nothing can reach it, cool what already burned, and give it the time it needs to rebuild.

Once I understood that the problem was an enzyme sitting on unprotected tissue, three things had to happen together.

Cover the raw tissue, so the lining gets a night off instead of another burn.

Calm what is already damaged, because swollen tissue cannot rebuild.

Heal the lining, because tissue repairs itself out of what you give it, and nothing repairs on an empty tank.

I use a daily liquid called EsoRepair.

The EsoRepair bottle standing on a kitchen counter
Sixty millilitres. Thirty servings. It lives on the kitchen counter.

It is a liquid rather than a tablet on purpose.

A tablet is built to get past your throat fast and dissolve in your stomach, and your throat is the part that is hurt.

A liquid coats it on the way down.

Cross section of two throats, a tablet falling down the middle of one and a liquid coating the walls of the other
One drops straight past. One touches the walls on the way down.

There are seven things in it and each one does one of those three jobs.

Here is exactly what is in one serving. Aloe vera leaf extract at 300 mg. Licorice root extract at 250 mg. Turmeric root extract at 120 mg. Grape seed extract at 100 mg. Vitamin C at 80 mg. Vitamin E at 20 mg. Vitamin B6 at 10 mg.

Aloe vera leaf extract and licorice root extract are the coating.

Both turn slippery in liquid and cling to what they touch, which is the entire reason this is a liquid and not a tablet.

Turmeric root extract and grape seed extract are the calming part.

Both are plant antioxidants.

Vitamin C, vitamin E and vitamin B6 are the rebuilding part.

Your body already uses all three every day to keep normal tissue healthy.

Vitamin C is the one it uses to build the springy fibres that hold soft tissue together.

The weave underneath is what holds soft tissue together.

The aloe is the piece with a proper trial behind it.

In 2015 a team led by Yunes Panahi published one in the Journal of Traditional Chinese Medicine.

Seventy nine people with reflux, four weeks.

Aloe vera syrup tested straight against omeprazole and ranitidine.

The aloe brought down all eight of the main symptoms they were measuring.

I will be honest about the limits.

That trial was on the heartburn kind of reflux, not the throat kind, and it used more aloe a day than one serving of this gives you.

It tells you the plant does something.

It is not a study of this bottle.

One more thing, and it matters if you take tablets for anything.

The licorice in it is the whole root, with nothing taken out of it.

Whole licorice can push blood pressure up in some people.

If you take anything for blood pressure or your heart, check with your doctor before you start.

I take it in the evening, which is the dose that matters most to me, for reasons that are probably obvious by now.

The slow swallow is the whole point. That is what puts the layer where it is needed.

That is the entire thing you have to remember.

Two millilitres is a little under half a teaspoon, so it is over in a second and there is nothing to swallow.

A teaspoon holding only a small pool of dark amber liquid beside an unlabelled dropper bottle
That is the whole serving.

Questions People Ask Before They Try It

Will it help me sleep?

Not directly, and I want to be honest about that.

It is not a sleeping tablet and there is nothing in it that makes you drowsy.

It works on the thing waking you up, which is a different job.

Do I still have to sleep propped up?

Do whatever gets you through the night while you start.

I did not go back to the stack of pillows, but I did not throw them out on day one either.

Can I take it with sleeping tablets or my other medicine?

You do not stop anything to try it.

This is a food supplement, not a drug, and it is not an acid blocker, so it is not doing the same job as anything you were prescribed.

If you take prescription medicine, run it past your doctor first.

Why does it happen at night and not in the day?

Because of gravity.

Upright, what comes up tends to go back down.

Flat on your back, it settles on the soft skin at the top of your throat and stays there for hours.

Three panels linked by arrows showing pepsin landing on throat tissue, waiting asleep, then waking when acidic food arrives
This is why a meal on Monday can still burn on Wednesday.

I have tried not eating late.

So had I, and it helped a little and then stopped helping.

Not eating late gives your stomach less to send up.

It does nothing about what is already sitting on your throat from the night before.

Is there anyone who should not take it?

This uses whole licorice root, not the stripped-down kind.

Dried licorice root on a board, whole dark fibrous pieces on one side and pale stripped pieces on the other
Two different things sold under one name.

Whole licorice can push blood pressure up in some people.

If you take anything for blood pressure or your heart, check with your doctor before you start.

How Long It Actually Took

Two panels side by side. On the left the throat lining is red, rough and torn along its edge. On the right the same lining is pale pink and smooth, with an arrow carrying the eye from one to the other
A lining that is being hit every night, and the same lining once the hits stop. Tissue repairs slowly.

I am going to be honest about this because I nearly gave up.

Nothing happened in the first few days. If you want a pill that fixes tonight, this is not that, and anyone who tells you otherwise is selling you something.

Week one, I noticed the mornings first. Less scraping when I woke up.

Around week three, I had a full night.

One, then another a few days later.

That was the point I let myself believe it.

Week six to eight, the wake-up in the small hours had mostly stopped, and the third pillow came off the bed.

That last one sounds small and it was not.

I had had three pillows on that bed for four years.

This is why it is a daily thing and not a when-it-is-bad thing.

Your throat lining is trying to rebuild, and it can only do that if the conditions hold night after night.

I wrote it down, because I did not trust my own memory about whether anything was changing.

Week one. Nights the coughing woke me — 5 out of 7.

Week three. 3 out of 7.

Week seven. 1 out of 7.

Those are numbers out of a notebook, not out of a study, and they are mine rather than anybody else’s. But they are the whole reason I kept going past the first month, when honestly I could not tell.

The thing that finally convinced me was not my own list.

It was my husband saying, about nine weeks in, that he had not heard me at two in the morning for a while.

He was not being kind about it. He genuinely had not noticed it stopping, which is exactly the point. The people who live with this notice the things we stop doing long before we notice them ourselves.

One tick a night. That is what 90 nights looks like on a kitchen wall.

Will It Actually Do Anything, Or Is This Another Thing That Does Not Work?

That is the right question and it deserves a straight answer.

It does not work in a day. Nothing that has to let a lining rebuild works in a day, and anything sold as working overnight is selling something else.

It is a nightly thing for 90 nights, because the throat only gets to repair if the nights hold. That is why the promise is 90 nights and not 30. 30 is not long enough to know, and asking somebody to decide at 30 is asking them to guess.

Stock is checked on the page itself rather than here, because it moves and this article does not. The link below goes to the current availability and the current price. There is no code to remember and no timer counting down on this page, because a timer that restarts for the next reader is not a deadline at all. The one honest thing the date at the top does tell you is that this page is still up. We take it down when a batch runs out, because sending people to a bottle we cannot ship is worse than sending them nowhere. Every batch so far has gone faster than the one before it.

So one honest warning before you go and look. This is a small operation and the bottles are made in batches. If you click and the page says there are none left, I am sorry. That does happen. It is still worth clicking to see for yourself, because the only way to know what is on the shelf today is to look at the shelf.
